Application for an examination after a prolonged absence

The employer and the employee are entitled to apply for a medical examination to be made by the appropriate occupational physician when work is resumed after an absence of more than six weeks because of illness or accident. Please complete the prolonged absence form and return it to us. We will then contact you to make an appointment

Application for dispensation for pregnant women

The application must be duly completed by the employer and handed to the employee before the medical examination by the occupational physician.
