Request for medical examination after extended leave
The employer and the employee are entitled to apply for a medical examination to be made by the appropriate occupational physician when work is resumed after an absence of more than six weeks because of illness or accident. Please complete the prolonged absence form and return it to us. We will then contact you to make an appointment.
Request for medical advice or exemption, for pregnant employee
The request must be duly completed by the employer and returned to the employee before the medical visit to the occupational physician.
